At Desert Retina Consultants, our goal is to make your experience safe, efficient and informative. Below you will find some helpful information about your upcoming appointment, including patient forms you may print and bring to the office if you choose. If you have any questions about your upcoming visit, please call our office at (760) 340-2394.

In addition to the forms below, please be sure to bring:

  • We recommend you bring someone to drive you home after your dilated eye exam
  • Insurance card or proof of insurance
  • Picture I.D. with current address
  • Name of your current pharmacy, address, and phone number
  • Co-payment or Deductible (payment is due at time of service)

Understanding which insurance applies to your eye care visit can feel confusing. That’s why we created this short video to explain the difference between health insurance and vision insurance, and when each one is used.

Both health and vision insurance play an important role in keeping your eyes healthy and your vision clear:

  • Health insurance generally covers medical eye exams and treatments related to eye health — such as glaucoma, cataracts, or eye injuries.
  • Vision insurance helps with how you see day-to-day. It often covers routine eye exams, prescription glasses, contact lenses, and other vision services.
  • Some plans (like Medicare) may cover certain medical services but not routine refraction tests for glasses prescriptions.
  • Even if adult vision benefits aren’t included, most health plans provide children’s vision services.

To make the most of your benefits, please:

  • Bring both your health insurance card and, if you have one, your vision insurance card to your appointment.
  • Provide a printed list of all current medications.
  • Ask our friendly staff any questions — we’re here to help.

What to expect at your visit

During your visit, a thorough examination and testing will be performed by our physicians and staff. This usually requires that your pupils be dilated. Dilation of the pupils typically lasts 2-6 hours and can impair your vision. While this is transient and variable in each patient, for safety reasons we recommend that you arrange for a driver to and from our office. We also welcome family or friends to attend appointments to help with questions or instructions.

Upon arrival to the office, you will check in at the front desk and receive forms to complete. You may download patient forms above to same time during your visit.

You will then meet with a medical technician who will thoroughly document your concerns and medical history. To help expedite your evaluation, we ask that you bring a list of your current medications and medical history.

Next, your ocular “vital signs” (i.e., vision and pressure) will be assessed. Your eyes will be dilated, and you will be asked to wait as the drops take up to 30 minutes to take effect. At this time, the doctor will screen your chart and order further testing to be performed while you wait. Depending on what needs to be addressed during your visit, your visit time to the office may last as short as 30 minutes, to as long as 2 hours or more. If you need to take specific medications at set intervals (particularly for diabetes), we recommend that you bring your medications with you.

After the preliminary work-up is completed, you will move to the exam room. Your doctor will perform an examination and interpret the findings. If necessary, specific imaging tests of your eyes may be performed on the same day or a different day to assist in the diagnosis and treatment of your condition. Your diagnosis will be reviewed and recommendations regarding treatment will be made. Most treatments can be performed in the office on the same day or re-scheduled as necessary. If surgery is required, after discussion with your doctor and addressing your questions and concerns, you will meet with a surgery scheduler to arrange the details of surgery.
